Current NYS License Registration *

NYS License Certificate *

NBRC, CRT/CRTT/RRT Certificate (Respiratory Services only) *

School Diploma *

Malpractice Insurance

Current BLS, ACLS, PALS, NALS card *

Current Health Certificate (physical exam, form can be picked up in the office)

Urine Drug Test (form can be picked up in the office, processing fee is required)

Criminal Background Check (done in the office, processing fee is required)

Social Security Card *

Proof of Citizenship (Birth Certificate, Passport, Resident Alien Card) or proof of
